As a nurse myself, I built my business based on the belief that entrepreneurship can bring professional freedom.

I see business as connected to nursing values, ethics, and wisdom. Business can give nurses more freedom, authority, creativity, and a chance to make a real difference, all while staying true to their professional identity. Nurses need business education made for them, with practical steps and support to help them practice well.

This is important because business and nursing come from different traditions. Business often follows a 'buyer beware' mindset, while nursing is built on service and public trust. According to Gallup, nurses have been rated highest in honesty and ethics for 25 years, with 75% of Americans rating them highly.

Nurses should not have to give up the qualities that make their profession trusted to build a business. Nurse entrepreneurs need business education that is evidence-based, trauma-informed, and respectful of everyone—not sales tactics or scripts that break trust.

Why conventional business advice breaks down for nurses

Typical business advice tells nurses to post more, be more visible, and push urgency before understanding their audience. These are hustle tactics built on shaky ground. More visibility and urgency do not help if people do not see themselves in a nurse business owner's message or value.

Many nurses start businesses carrying exhaustion, nursing-role conditioning, and distrust of business culture. When guidance feels generic, performative, or pressure-based, it does not just feel unhelpful. It feels fundamentally misaligned.

That advice does not work well for nurses and can be hazardous to their license and mental health.

Nurses need business education that helps them be understood before pushing them to be more visible. A polished message is not the same as a trustworthy one.

My approach starts with a different premise: Nursing is a disciplined, evidence-based, ethically grounded profession. Nurses should not abandon that foundation to build businesses. They need business education that helps them translate it into offers, communication, decisions, and models that fit who they are and how they want to serve.

The real question is not if nurses should go into business, but how they can build businesses that last and stay true to their values, helping clients, communities, and themselves without leaving nursing behind. The Holistic Caring Process, a key part of the Holistic Nursing Scope and Standards of Practice, gives clear guidance for nurse entrepreneurs.

What nurses need instead

Strong businesses are built on clarity, good judgment, and real evidence—not on guessing or copying others. This means noticing what is really happening, how people respond, and using that information to decide what to do next.

The six essential steps of the Holistic Caring Process—assessment, diagnosis, outcome identification, planning, implementation, and evaluation—can become a repeatable business development rhythm. Applied in business, they create a more humane and coherent path for nurse entrepreneurs.

In business, assessment starts with listening—not assuming, performing, or speaking out before you understand. Without assessment, outreach is just guessing. Assessment means asking about the challenges and opportunities a group faces and listening for patterns: what people struggle with, what they are tired of, and what better would look like to them.

Diagnosis in business means finding patterns and real needs, not just reacting to surface problems. For example, it might seem like you need more clients, but the real issue could be not understanding your audience, unclear messaging, or your message not reaching the right people.

Identifying outcomes is about integrity. It stops vague promises and keeps the nurse’s work realistic. This step means listening for the change the client wants, noticing progress or lack of it, and building a partnership with the client.

Planning, implementation, and evaluation should be more caring and people-focused than what is often seen in business.

A good plan matches the nurse’s capacity, the audience’s immediate needs, and the business's developmental stage.

Implementation does not mean trying to be everything to everyone. The goal is to show up in the right place with a clear message and a respectful approach.

Evaluation is not just about money or follower numbers. It also means checking if clients are making progress, if the work is sustainable for the nurse, and if the business stays true to its values.

Reflection and refinement: If a nurse feels scattered, unclear, or worn out by pressure to do more, the answer might not be more tactics. Instead, it could mean going back to assessment, finding patterns, setting clear outcomes, better planning, and honest evaluation. It could also mean showing more respect for both the client and the nurse’s autonomy.

Business can be a healing art

A well-designed business can give nurses more control, support their well-being, and help them serve in safe, lasting ways. If we want to heal the profession, we need more systems that provide agency, support well-being, and create safe spaces for nurses and those they care for. One way to do this is through businesses built by nurses and guided by the Holistic Caring Process.

Businesses are stronger when they are built on trust, clarity, and honest invitations—not pressure or vague promises. That is why I teach nurses to avoid business and marketing models that use pressure, manipulation, urgency, or ask them to ignore their own needs. I do not believe sales should be a place where normal ethics do not matter.

Ethical business means telling the truth about fit, offering enough information for a person to make a well-informed choice, and respecting the client’s autonomy rather than trying to override it.

Success is not only about what sells. It is about what lasts and supports both income and integrity over time. Nurses do not need scripts that take away their judgment or ask them to market in ways that do not fit their values. They need clearer thinking, better education, and practical, evidence-based ways to respond to what is happening.

My goal is to teach business in a way that helps nurses go from being seen as replaceable helpers to empowered professionals. I want nurses to build businesses that give them more freedom without sacrificing their judgment, professionalism, or trust. Nursing identity, ethics, and wisdom are not barriers to entrepreneurship—they are the foundation.

I want business education for nurses to match the high standards of the nursing profession.

If you are a nurse who wants to build a successful business without giving up your values, ethics, or professional judgment, I invite you to join my email list and receive Tips, Tools, and Wisdom for Liberated Nurse Entrepreneurs.

You will get practical advice, thoughtful insights, and evidence-based support to help you make better choices about your message, marketing, and next steps. There are countless nurse business ideas, and having a nursing education opens up even more possibilities. Nurses have business advantages over the general population due to our experiences, skills, work ethics, and public trust.

Nurse entrepreneurs use their professional nursing experience and education to start businesses in healthcare and other business sectors. Businesses established by nurse entrepreneurs may include developing and selling a health product or medical device or offering independent nursing services, such as health coaching, health education, home health, and many others. A nursing career path that includes entrepreneurship offers independence and autonomy and requires creativity, hard work, and strong business skills.

When nurses are successful in their own businesses, they can increase their income, satisfaction, and freedom to even higher levels than when they were employees.

Becoming a Nurse Entrepreneur

Nurse entrepreneurs work for themselves, so the skills required vary depending on the nature of their business venture. In general, the following skills and qualifications are beneficial for nurse entrepreneurs:

A valid RN license

Prior professional nursing experience in the business area of focus

Basic business skills and knowledge (e.g., accounting, marketing, and sales)

Good communication skills for interacting with customers, patients, and employees

Ability to work independently and willing to take on personal financial risk

What Are the Education Requirements for a Nurse Entrepreneur?

In general, nurse entrepreneurs have completed at least an ADN or BSN degree and attained an RN license in their state, but some business opportunities are also open to LPNs/LVNs. Business education is highly valuable and may include informal training or mentoring in business skills, taking a few courses in business, or attaining a business degree.

Are Any Certifications or Credentials Needed?

No specific certifications or credentials are required to become a nurse entrepreneur. However, depending on the nature of their business, certain certifications may be valuable.

The day-to-day responsibilities of a nurse entrepreneur depend on the specific venture but usually include activities related to running a business, such as accounting, marketing, sales, and developing a customer base.

Currently, more than half of all nurses are considering leaving their employment because of high stress, mental and emotional exhaustion, being buried in paperwork, or a lack of job satisfaction. Starting a business can help alleviate these issues and give nurses more autonomy and flexibility.

Note: If you search for nurse business education opportunities online, you will find many terms describing nurse business owners, including nursing consultants, self-employed nurses, nurse entrepreneurs, independent nurse contractors, independent registered nurses, and nurses in private or group practice. One company that provides nurse business coaching and education for all of those types is New Directions for Nurses, dba LiberatedNurse.com.

First, Check Your State’s Nurse Practice Act

Before you invest time or money into starting a nurse business, check your state’s Nurse Practice Act (NPA) to discover if what you want to do is within your legal right as a nurse. In some cases, nurses are more limited than the general public in their business activities.

The American Holistic Nurses Association (AHNA) has a web page that gives nurses access to the Nurse Practice Acts in all 50 states, and they update it every year. Check your state’s NPA here: https://www.ahna.org/Home/Resources/State-Practice-Acts

If your practice does not fit within the definition and scope of practice for a nurse in your state, you may be violating the law. The definition of nursing differs from state to state, but you will usually find it near the beginning of the Act.

If you are not 100% clear that your business activity is allowed, consult a lawyer who is familiar with nurse-owned businesses. Check with the National Nurses in Business Association (NNBA) for recommendations and other valuable nurse business resources.

What Are Your Interests and Skills?

Selecting the nurse business ideas that are best for you requires evaluating your skills, goals, and interests. To narrow the field of possibilities, ask yourself if you enjoy and are good at any of these services:

Educating others about medical or health-related issues

Designing and implementing wellness programs for individuals or groups

Identifying problems and solutions in healthcare organizations

Providing companion services

Providing medical equipment and supplies

Reviewing care provided by others and creating patient safety and well-being programs

Providing a nurse’s analysis of healthcare situations to lawyers as a nurse consultant

While both services cater to individuals needing assistance, home health focuses on medical care provided by nurses and therapists, while home care offers non-medical support through licensed care aides. Still, it is unclear what it means for a nurse to be entrepreneurial.

Most of us agree that today’s health challenges require new ways of thinking and acting. Entrepreneurial nursing offers enormous potential to contribute to holistic solutions and elevate the nursing profession’s identity and agency.

However, evidence shows nurse entrepreneurs’ abilities to engage in empowered entrepreneurship are often compromised 1, and very few nursing education programs exist to change that.

This article presents recent studies that contribute to our understanding of nurse entrepreneurship and a holistic nursing CE program created to show nurses how to grow thriving, sustainable businesses.

What Is Entrepreneurial Nursing?

An entrepreneur is an individual who creates a new business while bearing the risks and enjoying the rewards. Entrepreneurs play a key role in the economy by bringing beneficial new ideas to the market.

Entrepreneurial nurses are often described inconsistently by terms not founded in research. For the purpose of this article, entrepreneurial nursing refers to nurse entrepreneurs, who create their own businesses and nurse intrapreneurs, who create innovative solutions for their employers. Neither of these roles is currently recognized by the ANA. There is a need to examine entrepreneurial nursing roles and characteristics and the preparation, skill development, and support entrepreneurial nurses need to be successful.

My literature search failed to identify the number of entrepreneurial nurses. According to the U.S. Bureau of Labor and Statistics 2, registered nurses held about 3.1 million jobs in 2021. Their largest employers were:

Hospitals; state, local, and private

60%

Ambulatory healthcare services

18%

Nursing and residential care facilities

6%

Government

6%

Educational services; state, local, and private

3%

Other (including nurse entrepreneurs)

7%

Michelle Podlesni, RN, President of the National Nurses in Business Association, estimates that the current number of full-time nurse entrepreneurs in the USA is between 30 and 40 thousand, and a higher number are part-time entrepreneurs 3.

In 2021, a systematic literature review of 647 documents explored the main characteristics of entrepreneurial nursing. These characteristics were employment status, context, knowledge, activities, barriers, and motivations 4. Neergard found that entrepreneurial nurses’ roles are empirically rooted in the field of nursing and theoretically rooted in the field of entrepreneurship.

Additionally, she found that entrepreneurial nurses are influenced by their challenges in the healthcare system and the support they do or do not get. She offered that it is important that institutional nurse managers learn to identify entrepreneurial nurses’ characteristics and encourage them to develop their ideas.

Challenges and Opportunities for Nurse Entrepreneurs

Risks During Start-up

According to data from the Bureau of Labor Statistics (BLS), approximately 80% of small businesses survive the first year, 70% survive two years, but only 50% survive five years, and only 33% survive for 10. Healthcare and social assistance businesses have 5-10% higher survival rates than the mean 2.

Understanding what risks can lead to business failure and how each can be avoided or managed is vital. According to Horton 5, small businesses often fail from

Running out of money: Lack of clarity on how much net revenue is occurring and how to manage cashflow fluctuations

Inexperience managing a business: An unwillingness to delegate, a faulty business model, or a poorly visualized business plan

Unsuccessful marketing initiatives: Poor return on time and money invested, leading to #1

New nurse entrepreneurs need to balance their risk profile to protect themselves in the event of business failure. Assuming a 20-50% failure possibility in our first five years, we would be wise to distribute our time, energy, and money investments accordingly. Knowing this, many nurse entrepreneurs develop their business ideas while working as employees of others.

Internal and External Challenges

Jakobsen, et al.1 explored the experiences and perspectives of nine Danish nurse entrepreneurs. They found they faced many internal and external conflicts that compromised their ability to engage in empowered entrepreneurship. The results were organized into four themes:

Prejudice against entrepreneurship

Becoming an entrepreneur in a nursing culture

Rebellion against the conventional role of an employee

Challenging professional identity and forming new professional roles

They concluded that nurse entrepreneurship

Requires a significant learning process that develops nurses’ abilities to think outside the box

Has the potential to significantly develop the nursing role and identity and contribute to solving health challenges that require new approaches

Vannucci’s study6 aimed to understand the experiences and challenges of 44 nurse entrepreneurs who reported on their:

Transitions from employment to entrepreneurship

Key motivators in the decision to start a business

Challenges as entrepreneurs in the healthcare field

The study found that psychological empowerment factors were more motivating than financial gain or organizational constraints. The top psychological empowerment factors were

Meaning / purpose

Having an impact

Need for growth

Making decisions

The participants reported higher rates of self-care practices than a norm community sample, and while some work/life balance challenges were identified, the biggest challenges were identified as implementing a marketing strategy, networking, and accessing mentorship.

Ethical concerns also contribute to inner conflicts nurses experience. A recent study of 11 nurse entrepreneurs found they were particularly concerned with ensuring their ventures do no harm.7 My experience talking with hundreds of nurse entrepreneurs confirms that fear of harming people often deters them. These findings point to the need for educational pathways through which nurses can develop business skills aligned with nursing’s moral and ethical values.

Opportunities

According to the American Holistic Nurse Credentialing Corporation (AHNCC), holistic nursing aims to heal the whole person, grounded in the inherent interconnectedness of self, others, nature, and spirituality. Holistic nurses are instruments of healing and facilitators in the healing process, honoring the patient/client’s subjective experience of health, health beliefs, and values and viewing the whole person and his/her needs in their entirety, with integration as the goal8.

Emerging from holistic nursing, holistic nurse entrepreneurship offers enormous potential to contribute to holistic solutions and elevate the nursing profession’s role, power, and identity. I would love to see BSN nursing programs introduce foundational concepts of entrepreneurial nursing and develop that knowledge base in advanced-level programs.

My Point of View

I’ve learned from more than 40 years as an entrepreneur that small business development follows a fairly predictable process with specific developmental milestones to be met in each stage.

The Holistic Nurse Business Development Process™

When nurses learn to use this process, they feel more satisfied with their careers and become more empowered contributors to solving the holistic problems that individuals, groups, communities, and environments face.

To actualize their potential, entrepreneurial holistic nurses need evidence-informed holistic business education and mentoring designed by nurses. For example, most conventional marketing and sales education involves some level of pressure or manipulation to get someone to say yes - even much of what is called heart-centered, conscious marketing education.

I believe we don't have to choose between ethics and effectiveness. We don't have to get good at 'pitching' or do things that feel like we're selling our souls. In fact, we can let go of 'going for the sale' entirely.

What can we do instead? We can acknowledge the truth of whether or not our offer is a fit between us and the one thinking of buying. However, we need at least three things:

An evidence-based framework and map to guide our decisions

A toolset that fits our values, and

Insightful feedback to help us develop our skills

If we don’t have all three of these things, we’ll take the long and exhausting road to success, and too many of us will fail.

In the Liberated Nurse Entrepreneur programs at New Directions for Nurses, we teach marketing based on informed consent. This means we facilitate a caring, ethical, and conscience-led win-win for ourselves and the people we serve in every marketing and sales activity.

The Holistic Nurse Business Development Process™

The Holistic Nurse Business Development Process (HNBDP) helps nurse entrepreneurs succeed in their businesses by developing their holistic perspective, challenging and expanding the nursing culture and roles, and gaining confidence in evidence-informed business experimentation and value analysis.

The HNBDP involves translating conventional business processes into the Holistic Caring Process. Balancing profit motives with equitable access to healthcare is a primary concern. Nurse entrepreneurs’ ethics require them to prioritize the well-being of patients and communities, rather than solely focusing on financial gain. This means considering the affordability of their products or services and working to make them accessible to underserved populations.

A critical ethical consideration is ensuring that their ventures do not harm. Nurse entrepreneurs must uphold the principles of beneficence, non-maleficence, respect for autonomy, and justice in their business practices. This means adhering to professional standards, obtaining informed consent from clients, and avoiding actions that could harm individuals or communities.

Another challenge nurse entrepreneurs must address is overcoming perceptions that entrepreneurship conflicts with nursing values. Some nurses may view entrepreneurship as incompatible with the traditional values of nursing, such as altruism and caring. Nurse entrepreneurs can challenge these perceptions by demonstrating that entrepreneurship can be a powerful tool for advancing nursing practice and improving patient outcomes. By focusing on these areas, nurse entrepreneurs can build the necessary skills and knowledge to transform their healthcare expertise into successful business ventures that address unmet healthcare needs.
